If you have recently moved into the area and wish to join Len Valley Practice, firstly, please check that your address is within out Practice Catchment Area:

If you do not live within our Practice Catchment Area, you will not be allowed to register as a patient with us.

To be eligible to register at the practice, you must live within the practice catchment area.

 

Registration

If you do live within our Practice Catchment Area, we need you to print out the following form to bring into the surgery with you.

At this point you will be registered with us. 

If you are on regular medication, you will be given a New Patient Check appointment.

Homeless

If you are homeless and have no fixed abode and in need of urgent medical support, you will be seen at Len Valley Practice as an Immediate and Necessary patient and can register with the practice without any ID or address.

 

Non English Speakers

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
