ANNUAL HEALTH REVIEWS

Having a single annual review is the quickest and most convenient way of monitoring long-term conditions. Without regular check-ups, conditions can worsen and complications might arise that damage your health.

To ensure that patients with long-term conditions only have to attend the Practice when necessary, we now invite these patients to attend the Practice for a single annual review in their birthday month.

This may require an initial appointment with a Health Care Assistant (HCA) for blood tests, and checks of height, weight, blood pressure, and a urine test, usually followed by a face-to-face review with one of our practice nurses. In some circumstances the review can be completed simply with a telephone call to discuss the test results.

You will receive an invitation in advance of the planned review to allow time to book the necessary appointments. The conditions for which annual reviews are undertaken include:

  • Diabetes, including those at high risk of developing diabetes
  • Heart disease
  • Stroke
  • Hypertension (high blood pressure)
  • Atrial fibrillation
  • Kidney disease
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Rheumatoid arthritis
  • Long term mental health problems

Medication review dates will also be updated once the annual review(s) has been fully completed. 

The invitation will be sent via text or letter. This will include everything that you will need to do for your annual review.
