Len Valley Practice – Patient Charter

At Len Valley Practice, we are dedicated to providing high-quality healthcare in a safe, supportive, and respectful environment. We value the trust you place in us and are committed to working in partnership with our patients.

To help us deliver the best possible service for everyone, we kindly ask that all patients follow the principles outlined below. These are based on mutual respect, clear communication, and shared responsibility.

What We Ask of Our Patients

1. Be Respectful

Please treat all members of our team – including receptionists, nurses, clinicians, and support staff – with courtesy and kindness.
Everyone at Len Valley Practice is here to help you. Our staff often work under pressure and under the guidance of our GPs to ensure your care is delivered smoothly.

2. Communicate Openly and Politely

Our reception team may ask you questions about your symptoms or concerns. This is to ensure you are directed to the most appropriate clinician or service.
We ask that you respond clearly and politely, so we can help you as efficiently and safely as possible.

3. Attend Appointments Responsibly

If you are unable to attend your appointment, please let us know as early as possible so we can offer the slot to someone else.
Missed appointments waste valuable NHS resources and may delay care for others in need.

4. Book Appointments Appropriately

If you have more than one concern to discuss, please ask for a double appointment when booking. This ensures your issues can be addressed thoroughly without overrunning the allocated time.

5. One Patient Per Appointment

Please book a separate appointment for each family member needing medical attention. This helps us manage time effectively and ensures every patient gets the care they need.

6. Use Our Time Wisely

Where possible, book in with the clinician best suited to your need — such as a nurse, healthcare assistant, or pharmacist — rather than defaulting to a GP. This helps us provide timely access to the right care for everyone.

7. Keep Us Updated

Let us know promptly if your contact details change or if your personal circumstances may affect your healthcare. Keeping our records accurate helps us provide safer and more effective care.

Working Together

By following this charter, you help us create a respectful, efficient, and caring environment for all patients and staff.
Thank you for being a valued part of the Len Valley Practice community.
